Local forms are drafted and approved by the Board of Judges for use in the District of New Jersey.  If a form use has been designated as mandatory, failure to utilize the form may result in a delay in the processing of a case.


  • Official Bankruptcy Forms Official forms are approved by the Judicial Conference of the United States and are mandatory. They must be used to file a bankruptcy petition and take certain other actions in a bankruptcy case, such as filing a Proof of Claim or applying to pay a filing fee in installments.  
  • Director's Forms Director's forms are drafted and approved by the Administrative Office of the U. S. Courts. They are recommended for use, rather than mandatory.
